This is a version of the Zung Self-Rating Anxiety Scale. It was designed by William Zung, a psychiatrist from Duke University.

For each question you should choose the answer that best describes how you have felt for the past week. There are 20 questions but it should not take you longer than 5 minutes to go through it. You should not think too much about your answers, go for the first thing that comes to your mind.

The scale is NOT a substitute for a comprehensive evaluation and diagnosis by a psychiatrist. It is only a screening tool that may help identify the need for further investigation.
